One number drives the whole table: tadalafil’s half-life is about 17.5 hours. That’s why a daily 5 mg dose works at all, and why on-demand 20 mg lasts into the next day. Neither strength creates arousal.

Daily 5 mg: the five-day ramp and what “always on” means

One 5 mg tablet gives a low blood level for a day. The next day’s tablet lands on top of what’s left, and so on. The bigger the pack, the lower the price per tablet, and the 360 (delivery included, like every pack) is a year of medication for the price of four and a half monthly boxes. Centurion’s expiry is usually two to three years out, so a year’s supply is safe.

Side effects and missed doses on each routine

The side-effect list is identical because the molecule is; what changes is how often you meet it. On daily 5 mg, headache is mild if present and often fades after the first week or two; flushing, blocked nose and indigestion are rare; the back-ache that is tadalafil’s signature is uncommon at this dose. On on-demand 20 mg, headache on the day is common, and back or muscle ache turns up 12–24 hours later in a fair number of men and lasts a day or so.

The difference that actually matters is the nitrate window. On 20 mg, the interaction risk lasts about 48 hours after each dose. On daily 5 mg you are never clear of it. If you’re ever given a nitrate — a GTN spray in an emergency department, say — the doctor must know you take tadalafil every day. Tell them.

Never with nitrates, on either routine. GTN sprays or tablets, isosorbide mononitrate or dinitrate, nicorandil, and “poppers” (amyl/butyl nitrite). The combination can crash blood pressure. And an erection lasting more than four hours is an emergency — hospital, not sleep.

Missed a daily dose?

Take it when you remember, unless it’s nearly time for the next one — then skip it and carry on. Never take two to catch up. Because the half-life is long, one missed tablet barely dents your blood level. Miss three or four in a row and you’re restarting the five-day ramp.

On 20 mg there’s no such thing as a missed dose, only a badly timed one. If the evening didn’t happen, the tablet is still working tomorrow. One per 24 hours.

Which routine fits you
